The Agent’s Guide to Life Insurance Underwriting for TRT and Anti-Aging Clinics
Stop assuming hormone therapy is a red flag. Here is how underwriters actually view TRT, peptides, and wellness clinics today.
You are doing a fact-finder with a 45-year-old male client. He is in the best shape of his life, exercises five days a week, runs a successful business, and wants a $5M term policy to protect his family and his company.
It looks like a slam-dunk Preferred Best case. Then, he mentions that to keep his energy levels up, he visits a local wellness clinic for weekly Testosterone Replacement Therapy (TRT) and peptide injections.
A decade ago, an agent might have panicked, assuming the carrier would view this as illicit steroid abuse or a major medical red flag. Today, TRT and anti-aging clinics are a massive, booming industry—particularly among high-income men aged 35 to 55, which happens to be the exact demographic buying the largest life insurance policies.
Because this has become so mainstream, underwriters have completely adapted. A client on TRT does not automatically lose their top-tier rate class. In fact, if managed correctly, they can easily secure Preferred or Preferred Best rates.
Here is exactly what underwriters are looking for when your client mentions a wellness clinic, and how you should field-underwrite these cases.
1. Legitimacy: The “Doctor Supervision” Rule
The absolute most important distinction for an underwriter is where the client is getting their testosterone.
-
The Green Light: If the TRT is prescribed by a licensed physician or a legitimate wellness/longevity clinic, and the client is having their bloodwork monitored regularly, underwriters view this as standard medical maintenance.
-
The Red Light (Automatic Decline): If the client is “self-prescribing,” buying testosterone online, or getting it under the table from a buddy at the gym, the carrier will immediately decline the application. Unsupervised hormone use is viewed as reckless behavior and a massive mortality risk.
2. The Bloodwork: Hematocrit & PSA Levels
When a client is on TRT, the underwriter is going to look incredibly closely at their paramedical blood panel—specifically two major markers:
-
Hematocrit (Blood Thickness): Testosterone naturally stimulates the production of red blood cells. If the dosage is too high, the blood can become too thick (high hematocrit), which drastically increases the risk of blood clots, strokes, and heart attacks. As long as the client’s doctor is monitoring this and keeping their hematocrit in a normal, healthy range (often by having the client donate blood occasionally), the underwriter is happy.
-
PSA (Prostate-Specific Antigen): Testosterone can stimulate prostate growth. Underwriters will check the client’s PSA levels to ensure the TRT isn’t masking or accelerating prostate cancer. Again, a normal, stable PSA means the client is clear for top-tier rates.
3. The “Why”: Hypogonadism vs. Anti-Aging
Underwriters will always look at the underlying reason for the prescription.
-
If the client was diagnosed with Hypogonadism (clinically low testosterone) by an endocrinologist, it is a straightforward medical condition that is easily underwritten.
-
If the client is using it purely for Anti-Aging or Athletic Recovery, the underwriter will want to ensure the dosages are therapeutic and not bordering on performance-enhancing bodybuilding levels.
4. What about Peptides and HGH?
Wellness clinics often bundle TRT with other injectables, which agents need to watch out for.
-
Peptides (like BPC-157 or Semaglutide): Generally, underwriters are fine with these as long as they are prescribed for legitimate reasons (like weight loss or joint recovery) and the client has no severe underlying diseases.
-
HGH (Human Growth Hormone): This requires much stricter scrutiny. Unless the client has a very specific, diagnosed pituitary deficiency, underwriters are highly suspicious of HGH prescriptions. If your client is taking HGH purely for anti-aging, it will likely bump them out of Preferred rates and require a highly specialized carrier to get approved.
Your Field Underwriting Checklist
Before you drop a formal application for a client using TRT or visiting an anti-aging clinic, you must ask these four specific questions:
-
Is this legally prescribed by a doctor or legitimate clinic? (Get the name of the clinic).
-
How long have you been on this treatment, and what is your exact dosage?
-
How often does your doctor run bloodwork to check your levels? (The answer should be at least 1-2 times a year).
-
Are you taking any other injectables, like Peptides or HGH? (If yes, get the exact names).
Stop Guessing. Let Us Pre-Screen the Case.
You do not want to surprise an underwriter with a TRT disclosure on a $5M application, only to find out that specific carrier has strict internal rules against it.
If you are working with a client who utilizes hormone replacement therapy, do not submit a formal application blindly.
Instead, send our brokerage team the answers to the four questions above. We will run a completely anonymous pre-screen with our underwriting desk, review their baseline numbers, match your client to the most favorable carrier, and help you lock in a Preferred Best rate.
